Phyllis K Provenzano

Phyllis Kathryn Provenzano Phyllis Kathryn Provenzano of Toledo, Died December 28, 2013 in the Swan Creek Health Care Center at the age of 92. She was born in LeMars, Iowa to Henry Alvin and Hilda Paulyn DeJong Heusinkveld. She earned degrees from Northwestern Jr. College and Rush Presbyterian School of Nursing (Registered Nurse) in Chicago IL. In 1944 she married Joseph A. Provenzano, MD (predeceased) They moved to Toledo in 1947 following his army service in WWII. She is survived by their children: Jeanne Schafenberg, Dover, DE; Gordon (Karen) Provenzano, Saginaw, MI; Fredric (Margaret) Provenzano, Seattle, WA; and David (Joan) Provenzano, Wauseon, OH; four grandsons; Adam (Erlyn) and Matthew (Dreme) Schafenberg, Joel (Laura) Provenzano, and Mark Provenzano; four granddaughters; Ann (Brian) Provenzano Klerman, Jennifer (Gannon) Provenzano Mooney, Stacy (Ryan) Provenzano Nowak, and Kelly Provenzano; six great grandchildren; Kealy, Kate, Matthew, Elliott, Audrey, and Joseph; brother Wayne Heusinkveld; four foreign exchange students. Phyllis worked in her husband's medical office for over 20 years. She was always active in her church; Collingwood Presbyterian Church, then Christ Presbyterian Church as an Elder, choir member, missions, and teacher. With her husband she served on several short term medical missions in the USA, twice in Costa Rica and did sickle cell testing. She was also active in her children's lives as room mother, Cub Scout den mother, PTA, AFS, YMCA and fund raising for the Medical Mission Ship "Hope". She and her husband moved to Swan Creek Retirement Village in 1996. Proceeding her in death were her parents, brothers Fredrick (infancy), Milton, LeRoy, and Irwin and grandson Joseph Provenzano. Services will be held at Christ Presbyterian Church on Saturday January 18, 2014 at 11:00 A.M. .
